How gum disease progresses
Plaque bacteria at the gumline cause inflammation — gingivitis — which is reversible. Left unchecked, the inflammation extends deeper, forming pockets between gum and tooth where bacteria are protected from your toothbrush, and the bone anchoring the tooth begins to resorb.
That bone loss is permanent. The aim of treatment is to arrest it before teeth become mobile.
Non-surgical treatment first
Most cases are managed non-surgically: careful debridement of the root surfaces below the gumline, sometimes over several extended sessions and under local anaesthetic, plus a genuine change to home cleaning. Locally applied antimicrobial gel is used in selected deeper pockets.
Progress is measured by re-charting pocket depths, not by how the gums feel.
Surgical options and maintenance
Where deep pockets persist despite good non-surgical treatment and good home care, surgical access to clean and reshape the affected area may be considered, or referral to a specialist.
Whatever the route, periodontal disease is controlled rather than cured. Lifelong maintenance appointments, usually three to four monthly, are what keep it stable. Smoking substantially worsens the outlook.

Can gum disease be cured?
