Who we are
Wolverhampton Dental is a dental practice at 21 Raynor Road, Fallings Park, Wolverhampton WV10 9QU. We are the data controller for the personal information we hold about our patients. You can contact us on 01902 723 400 or at info@wolverhamptondentist.co.uk.
What information we collect
Contact details such as your name, address, telephone number and email address; your medical and dental history; clinical records, radiographs, photographs and scans taken as part of your care; details of treatment provided and payments made; and correspondence with you or other healthcare professionals involved in your care.
Why we use it
We use your information to provide safe dental care, to contact you about appointments and recalls, to process payments and insurance or plan claims, and to meet our legal and regulatory obligations to bodies such as the Care Quality Commission and the General Dental Council.
Our lawful bases are the provision of health care and treatment, our legitimate interests in running the practice, and compliance with legal obligations. Marketing messages are only sent where you have given consent, and you can withdraw that consent at any time.
Who we share it with
We may share information with other healthcare professionals involved in your treatment, such as specialists we refer you to, dental laboratories, and where relevant your dental plan provider or insurer. We do not sell your data. Where a supplier processes data on our behalf they do so under a written contract.
How long we keep it
Adult dental records are retained for at least eleven years after the last treatment. Records for children are retained until their twenty-fifth birthday, or eleven years after the last treatment, whichever is longer.
Your rights
You have the right to ask for a copy of the information we hold about you, to have inaccurate information corrected, to object to or restrict certain processing, and in some circumstances to have data erased. To make a request, contact the practice in writing.
If you are unhappy with how we have handled your information, you can complain to the Information Commissioner's Office at ico.org.uk.
